Output a page as PDF (ES110913_outputpage_as_PDF)

The Output a page as PDF (ES110913_outputpage_as_PDF) sample app is an example of outputting a web page (Ebase form page) as a PDF.
It includes an example of dynamic behaviour (hiding and showing fields on the PDF as required) as well as adding headers and footers to each page. The footer contains page numbers, eg "Page 1 of 4"
